Registration Requirements

Tick Tock is licensed by the Illinois Department of Children and Family Services (DCFS) and therefore adheres to all rules and regulations as outlined by the department.  To enroll your child at Tick Tock the following documents will need to be completed and submitted for each child before your child can participate in our program:

        Registration Form

         Certificate of Child Health Examination* (Signed by Physician and dated within 6 months of registration if the child is not currently enrolled in school or dated within 1 year if the child is enrolled in school)

         Childhood Lead Risk Assessment for children age 6 and under (Signed by Physician and dated within 6 months of registration if the child is not currently enrolled in school or dated within 1 year if the child is enrolled in school) Zip Codes

         Policy and Permission Agreement

         Pricing and Payment Policy

         A certified copy of the childís Birth Certificate as mandated by The Missing Children Records Act

 

In accordance with The Missing Children Records Act we now require parents to provide a certified copy of the childís birth certificate or other reliable proof of identity and age of the child prior to enrollment.  If a certified copy of the birth certificate is not available, the parent or guardian must submit a passport, visa or other governmental documentation as proof of the childís identity and age and an affidavit or notarized letter explaining the inability to produce a certified copy of the birth certificate.  If the parent or guardian fails to submit proof of the childís identity we are required by law to notify the Illinois State Police or local law enforcement agency.

Items to be completed at the time of registration:

  • Acknowledgment of Summary of Licensing Standards for Day Care Centers
  • Payment of Annual Membership Fee

*If Immunization Record isnít applicable parent or guardians must provide proof of immunity according to requirements in 77 Ill. Adm. Code 690.50 of the Department of Health rules (http://www.idph.state.il.us)
